Field Trip Information

MANDATORY VOLUNTEER AGREEMENT

Before you volunteer for a field trip, you must complete the LCMSD Volunteer Agreement to confirm that you understand and agree to our volunteer expectations. This form must be completed annually.

FIELD TRIP CHAPERONES

All chaperones, including drivers, must complete the Chaperone Guidelines Form for each field trip they volunteer to chaperone. Chaperones must also have their drivers license scanned by the school office staff prior to the field trip.

FIELD TRIP DRIVER CRITERIA

Student safety is our top priority. We confirm that each field trip driver applicant holds a valid CA driver’s license and review their DMV Driver's Record prior to approval. Drivers with any history of serious violations (such as DUI, reckless driving, excessive speeding, license suspension, or revocation) or multiple moving violations within the last five years are not approved to transport students. In general, we may allow one minor moving violation, including a single cell phone violation, provided it does not reflect a pattern of unsafe driving. In the case of a cell phone violation, the driver is reminded of California’s hands-free laws and our expectation of strict compliance while transporting students. Any record that suggests ongoing risk, repeated violations, or impaired judgment will result in denial at the District's discretion. Denials will be reported via email from the District Office.

FIELD TRIP DRIVE APPLICATION INSTRUCTIONS

At least 10 days prior to a field trip, all field trip drivers must submit a Driver Authorization Form with the required documentation specified below. If approved, the driver will be added to the District Authorized Driver List for the entire school year at all school sites. This process must be completed annually.

  1. Prepare the following required documents for the Driver Authorization Form and save a scanned copies as a .pdf file:
    1. Order a copy of your DMV Driver’s Record for a $2.00 fee (plus credit card fee) here: https://www.dmv.ca.gov/portal/customer-service/request-vehicle-or-driver-records/online-driver-record-request/
    2. Copy of valid (unexpired) Driver’s License
    3. Copy of current insurance billing statement (within 6 months)
    4. Copy of current insurance policy declarations page (usually the first page), which shows coverage limits and dates of coverage.
  2. Submit a Driver Authorization Form and attach the .pdf file of your required documents.
  3. Complete the Chaperone Guidelines Form
  4. An Office Staff member will run your Driver’s License through the Raptor Visitor Management system.
  5. You will be notified of your status as an authorized driver within 10 days. If you are not approved, you will be informed of the reason for your denial.
  6. Upon approval, you will be added to the District Authorized Driver List (available to all teachers) for the school year.

FIELD TRIP LUNCH INFORMATION

Field trip lunches are offered to students at no charge when a class field trip overlaps the lunch hour. Field trip lunches are not ordered online, they are ordered by your classroom teacher 10 days before the actual field trip. Your classroom teacher will send out an email asking if any students would like a field trip lunch. The field trip lunch is a sunbutter and jelly sandwich with fruit and vegetable.
